Abstract

Latar Belakang: Asma adalah penyakit gangguan inflamasi kronik pada saluran
napas dan penyakit ini masuk kedalam sepuluh besar penyebab kesakitan dan
kematian di Indonesia. Polimorfisme yang terjadi pada gen ORMDL3 diketahui
memiliki hubungan dengan inisiasi sistem imun paru karena dengan adanya mutasi
maka gen ini akan gagal mengkode protein yang akan menjadi downregulator
sistem imun sehingga sistem imun menjadi hiperrepons terhadap allergen.
Polimorfisme ADAM33 juga menjadi dasar kuat kerterlibatan dalam patogensis
asma karena terdapat pada fibroblast saluran pernapasan sehingga dapat
menyebabkan remodeling saluran napas. Tujuan: Penelitian ini bertujuan untuk
mengetahui hubungan polimorfisme gen ORMDL3 dan ADAM33 terhadap kejadian
penyakit asma sebagai faktor risiko. Metode: Penelitian ini dilakukan dengan
metode observasional analitik dengan pendekatan case-control dan melibatkan 33
pasien asma dan 33 kontrol sehat yang kisaran usia disesuaikan serta jenis kelamin
disamakan. PCR-RFLP digunakan untuk memeriksa polimorfisme. Analisis data
dilakukan dengan chi square test dan odds ratio. Hasil: Analisis data menunjukkan
tidak adanya hubungan yang signifikan pada polimorfisme gen ORMDL3 terhadap
kejadian penyakit asma (OR 1.00, 95% Cl 0.060-16.690, P = 1). Hal yang sama
juga terjadi pada hasil polimorfisme gen ADAM33 gagal menunjukkan hubungan
signifikan dengan kejadian penyakit asma. (OR 0.431, 95% Cl 0.116-
1.604).Kesimpulan: Tidak terdapat hubungan yang signifikan antara polimorfisme
gen ORMDL3 maupun ADAM33 dengan kejadian asma di Cirebon.

Introduction: Asthma is a chronic inflammatory disorder of the airways and this
disease is one of the top ten causes of morbidity and mortality in Indonesia.
Polymorphisms that occur in the ORMDL3 gene are known to have a relationship
with the initiation of the pulmonary immune system because with a mutation this
gene will fail to code for proteins that will become immune system downregulators
so that the immune system becomes hyperresponsive to allergens. ADAM33
polymorphism is also a strong basis for involvement in the pathogenesis of asthma
because it is present in respiratory fibroblasts and can cause airway remodeling.
Aims: This study aims to determine the relationship between ORMDL3 and
ADAM33 gene polymorphisms on the incidence of asthma as a risk factor. Methods:
This study was conducted using an analytic observational method with a case�control approach and involved 33 asthma patients and 33 healthy controls who
were age-adjusted and sex-matched. PCR-RFLP was used to check for
polymorphisms. Data analysis was carried out with chi square test and odds ratio.
Results: Data analysis showed that there was no significant relationship between
ORMDL3 gene polymorphisms on the incidence of asthma (OR 1.00, 95% Cl 0.060-
16690, P = 1). The same thing also happened to the results of the ADAM33 gene
polymorphism which failed to show a significant relationship with the incidence of
asthma. (OR 0.431, 95% Cl 0.116-1.604). Conclusion: There is no significant
relationship between ORMDL3 and ADAM33 gene polymorphisms with the
incidence of asthma in Cirebon.
